Teacher Questionnaire

It’s hard to believe that summer is already over for some of you. Thankfully we have a couple weeks left and we hope to squeeze in as much fun as possible.
We are starting to prepare for the anticipated First Day of School though.
This year we decided we’d send a paper in for our teachers to fill out… I mean come on…  they send enough home for us to fill out each year. 🙂
Good news for them though… this worksheet will just make it that much easier for us to give gifts throughout the year that they really like and can use.

Does anyone else get a little overwhelmed with all those
 papers that get sent home on the first day?  🙂

    I am a teacher and LOVE this idea. I wouldn't mind filling this in. At our school (Netherlands) we don't have much papers to fill in for parents. Well sure when the child enters the school for the first time. But not every year again.
    But I do make a form like this for my class. Every year I have a different class as a mentorclass. So I have to do this every year. This year they are all boys. So I have to see what that brings.

    My son is starting K this year – first time going to the "real/big" school – I want to start crying now! Anyway – THANK YOU for this – I am definitely going to have his teacher fill this out!!! Looking forward to more of your exciting "teacher gift" ideas in the future! Thanks again!
